Pilot Program Application

Welcome to the AEFIS 3.0 Pilot Program Application Form.

To apply successfully, an authorized school or college administrator needs to take the following two steps. First, write a letter of support for your institution’s participation in the AEFIS 3.0 Pilot Program. We have provided a Letter of Support Template for your convenience. Second, complete the short form below in detail and attach the letter of support.

The letter of support can be scanned and uploaded as part of this form or send using one of the following methods:

  • Email - You can send scanned support letter(s) to pilot@goaefis.com.
  • Fax - You can fax support letter(s) to +1-215-873-0801
  • Mail - You can mail the support letter(s) to:

Pilot Program participants will be selected based on the details covered in the Pilot Program Guide document and the information provided in the application form. Only a handful of pilot program slots remain available and the deadline for applications is June 1, 2010. Applicants will be notified within two weeks of our receiving both the letter of support and the completed application below or by June 15, 2010 of their application status.
